Subject: Request for Maternity Leave
Respected Mrs. Mathews,
As I have mentioned earlier, I’m writing this letter as an application for a maternity leave, starting from the 1st of August till the 10th of November. I have attached a medical certificate confirming my pregnancy and my due date.
In the period that I will be absent, I have arranged for a substitute to take my place and also complete all my work on time. I have briefed her about the work and the deadlines. Please do not hesitate to contact me in case you need further details or if there is something else that I can do to ease the transition. I hope the University will cooperate with my substitute, Mrs. Martha Robins.
I really appreciate the provision that the office has made for pregnant women to take some time away from work for the birth of our babies and adjust to being a new mother.
Thank you for your support and I look forward to my return in November. Please pardon if any inconvenience is caused.
Yours sincerely
